Sim vs. Live

I had my account set to Sim trading for over a month now, and I just noticed my account # within the trade order window today confirming that I am set for Live trading when I never disabled the Sim mode. I'm ok with the hit. But it is very uncool that it happened and the consequences could of been much greater...Also within "Sim Mode", I can see my account # within the drop-down (not in dead gray like it used to be) which allows me to instantly switch from Sim to Live with no "Warning Box" asking for my permission which I have come very accustomed to over the many years. There is definitely a glitch going on here.

Even when in Trade Simulation Mode, non-simulated trading accounts will be listed. If you use one of them, the order would be rejected in this case.

The Trade Activity Log indicates when Trade Simulation Mode is enabled and disabled and the exact reason why:
Trade Activity Log: Viewing Historical Trade Activity

Make sure you display Info entries and set the Symbol list to "All Symbols (non-simulated)"

There should have been a prompt to confirm this:
which allows me to instantly switch from Sim to Live with no "Warning Box" asking for my permission

Once you find the relevant trade activity entry, we can then know what has happened.

Understood. But when I opened my platform that morning, it automatically switched to Live Mode and I got nipped several times before I noticed my account # was appearing. So I downloaded a new version and I will be very "Vigilante" going forward to avoid it from happening again.

We need to see the trade activity entry showing the reason for Trade Simulation Mode being disabled. The reason will be given.
