Even when in Trade Simulation Mode, non-simulated trading accounts will be listed. If you use one of them, the order would be rejected in this case.

The Trade Activity Log indicates when Trade Simulation Mode is enabled and disabled and the exact reason why:
Trade Activity Log: Viewing Historical Trade Activity

Make sure you display Info entries and set the Symbol list to "All Symbols (non-simulated)"

There should have been a prompt to confirm this:
which allows me to instantly switch from Sim to Live with no "Warning Box" asking for my permission

Once you find the relevant trade activity entry, we can then know what has happened.
