I have similar experience. Then I noticed, that the order remained in TWS with "Transmit" button next to it. It is really annoying to transmit every order... not possible for trading. I am on SIM, so I don't care yet, but I will soon... I guess, there must be a way to set transmitting without confirmation in TWS. I briefly looked at it and found some settings.
