Regarding the problem of not receiving automatic updates in the ntfy app, check the main homepage of the ntfy app and if you see "Reconnecting" from time to time, go to … >> Settings>> Connection protocol >> JSON stream over HTTP.

This does not appear to be a problem at the server level, but rather some unknown issue within the ntfy app or the phone operating system.
