If you have not exited from Sierra Chart since the last time this occurred, then start by looking in your "Trade >> Trade Service Log" and see if there is anything in there about why the Stop was not executed when you expected it to be. If you want to post the information from that log to this thread for us to take a look, then please do so. The next to do is look at the Trade Activity Log and make sure that the Stop Order was properly entered and at the price point you expected. And then look and see if there is any information about why it may not have filled. Make sure to check both Orders and Info.
