Make sure everything is setup for that particular chart Trade Window and that you are not viewing/modifying a Trade Window for a different chart. In terms of not being able to add an account, that would be due to that account being the selected account at that moment for that chart. Everything you show and state looks good and we do not see any reason for you to not have the trades being duplicated. After you open a trade, go to the "Trade >> Trade Activity Log >> Trades" and see if you see the trades for all the accounts you have setup.
