With Ninja Trader you should be setup to use either CQG or Rithmic for your order routing. Both of these are supported by Sierra Chart. Therefore, when filling out the Uniform Subscriber Agreement, you would enter "Ninja Trader" for the Broker/FCM and then enter your Ninja Trader Account Number for the Account Number. Also note that just being on Package 11 is not enough to get the real-time data. As you have noted, you need to fill out the agreements and activate the exchanges.
