Our apologies, we try to keep our documentation up to date, but somehow this got missed. You are correct, there are Pivot Points for Calculation Mode 0 at the 0.5, 1.5, 2.5, and 3.5 levels, which are just the average of the levels above and below. Note that there is NOT a level 4.5, so that is why you are not seeing it. We will update the documentation to reflect these other levels for that calculation mode.
