Attached take profit order remains active after stop is hit

I am trading with Interactive brokers and have been placing hundreds of orders like this through Sierra chart. In last few days, when my stop loss order is hit, my take profit order remains active? I asked IB and they said that they only see separate limit order sent to them... Do you know what is the problem?

Provide us the lines in the Trade Activity Log for the parent order and the Target and the Stop order. Here are instructions:

So there is no misunderstanding on our side, tell us the internal order IDs of these orders. We only need to see one set of orders where you had a problem.

What you have described in the first post did not happen.

The Target order did get canceled by Interactive Brokers:
