Live position disappearing and not showing in the trade window

I'm having a peculiar problem. When I place a live order, the orders show up (both market and limit, including attached limit orders) but a second or two after the order is filled it simply disappears. I found out today that when I hit the flatten button, a message comes up stating that the order does not exist. As a note, it will cancel any pending attached orders when flatten is used.
Chart Trade mode is on.
Show orders and position is on.
I am using IB and have it connected as I always had it in the past. I double checked and the set up seems fine.
I am using the Denali data via Sierra Charts (which is great)
Under "Symbol Action", I use STOCK Symbol-NQTV
Under "Trade and Current Quote Symbol", I use Stock Symbol-STK-SMART/NASDAQ-USD or Symbol-STK-SMART-USD (I tried both)

I tried to find a solution on my own, but nothing seems to work.

As a second question, it seems since changing to the "Symbol-NQTV" the only way to trade via IB is to individually go through each symbol and change the trade setting to Symbol-STK-SMART-USD. Is this correct? There is no way to change all tickers on an exchange en masse?

You may need to use the Order Fill Calculated Position setting.

Thanks for you reply. I am a little embarrassed that I couldn't find this section on my own. It never came up when searching.
Anyway. I believe I have solved it (at least temporarily). I watched the symbols on the Trade Position Window when placing an order today. In this case it was AMD. When the trade when through, the symbol came up in the Symbol Column, without the -STK-SMART-USD. So, I changed the symbol to just "AMD", in the section of "Trade and Quote Symbol (Action)" in the chart settings window. Surprisingly, this did transmit the order to IB and maintained the position in SC after the trade.
Thanks for your guidance and I hope this helps anyone else that has this problem.
