Let's say I placed an order with brackets and the stop loss target has been set to move the stop to 2 points take profit stop if 5 points has been hit. Is it possible to modify this after an order has been placed?

You can manually modify the order to place it at any particular point. But if you are asking if you can modify the settings for when the "Move to Breakeven" occurs, then the answer to that is no - you would have to remove the existing order and replace it with another order that has the new parameters in it.
