Order not filled with no order rejected message

Hi,

I was trading my live account (AMP, CGQ), had put a sell order in place with time enough, price went through the order and it didn't fill, I didn't get any message neither. As this is the second time that happens in the last 5 months, I would like to troubleshoot this issue, understand it and fix it if possible.

The order was a sell stop-limit at 3995.50 at around 9:58 am. Below some facts that may or may not be related:

- I trade the MES from an ES chart.

- The stop-limit order limit offset is set at 0 for both the primary and the attached orders.

- I, initially, always put the order far away from the price I want to enter, then drag and drop it in place once the candle closes. On occasion, if I am a bit slow, the order will be rejected and a message will pop-up (similar to "Trade Order Error - CQG order update. Status: REJECTED. Text: Sell order stop price must be below last trade price ExchangeRejectCode = 2060..."), I understand this. But it was not the case, I believe the order was in place with time enough and there was no message.

- After price crossed the order, I cancelled it fast because I didn't know what would happen if price came back and touched the order again.

I cheked the trade activity log but cannot see anything relevant, so I don't understand the issue. I attached the file.

Could you please support further in order to avoid reocurrence?

The general reason why this would happen:
price went through the order and it didn't fill,

Is because there is a zero offset for the Limit price:

- The stop-limit order limit offset is set at 0 for both the primary and the attached orders.
