Order exit issue

I am having an issue with order entry on exit. I always use an OCO entry with stop and target. Sometimes I exit before either are hit with FLATTEN on the chart trade window. What has started happening is my stop and target are cancelled, the position remains open until I hit FLATTEN a second time, and I get the error message below, which says I am doubling my size - I have deleted the amounts from the message as they don't matter.

If I use a keyboard short cut to flatten this doesn't happen.

Can you tell me what I need to do to stop this from happening.
