Problem with Stop loss distance and Trigger for Trailing Stop in Spreadsheet

1) I have set up a spreadsheet trading system and an attached order with a stop loss distance of 40 ticks.
If I run the system on a live market and the signal triggers an entry, the stop is initially correct on the chart, but after a while something triggers the stop randomly after 15, 20 or 21 ticks. How can this happen?

2) How do I set up a trigger for the attached order stop offset (J81) in the spreadsheet? At the moment the exit is defined in L3 when price hits let's say a MA or VWAP. I'd like to have the same signal to trigger the offset of the stop, not the exit. Could you give me a hint how to do this?

1. What do you mean by "triggers the stop"? Do you mean the stop order has filled?

We recommend examining the Trade Activity Log for that order to see all of the actions related to it:

1) It looks like the order has been cancelled. Can I upload the activity log for this trade? Because I can't read the problem in it. If it's cancelled I really don't know what the reason is.
