I can't for the life of me figure out which order to use to make the attached stop. I need a trigger price and a limit price. When I manually set it up with the broker it shows correctly. None of the orders on the drop down appear to be what I'm looking for. Stop limits don't have spots for two prices.

The second value for the Limit is set as a Price Offset from the Stop price. You do this on the Set tab of the Trade Window in the Stop-Limit Order Limit Offset section in the top of that tab. Refer to the following documentation for the Stop Limit order:
