An unmatched fill potentially could occur if you jump back in time during a chart replay and submit orders after that.

You need to delete the "Fills" and not the Orders Trade Activity to remove a trade from the Trades tab on the Trade Activity Log. This must be the problem. There really is nothing that can go wrong when deleting a fill.
