Your problem is the number of bars over which you are calculating the MIN and MAX. This is why you see the values look good for a bit close to the most recent bar, but then goes haywire prior to that. You need to calculate the Min and Max over the number of bars for which you want to normalize the data. Also, you should use an absolute reference for the MIN and MAX so you get the same value in the entire row.
