One of the problems you have is that you have not filled out the Exchange Agreements correctly. In order to qualify for the non-professional exchange fees, you need to have a futures trading account, and we need to know who that account is with and the account number in the Uniform Subscriber Agreement. Without this, you will be considered a professional and have to pay for the professional exchanges.
