First, you do NOT have to fill out the "Sierra Chart Order Routing Service Trading Account Assignment Request Form". Your trading account is already setup. Second, your account is setup to have all the fees that you list paid by you to AMP. This includes any cost for the Sierra Chart software, Denali Data Feed, and Order Routing Fees. Third, your account is not setup for Real-Time Exchange Data for the CME Group. If you want to have Real-Time data, then let AMP know to finish the setup for the Denali Exchange Data Feed for you. Be sure to let them know what exchanges you want to have activated.
