We are quite certain it is simply just a fill timing issue. It is not a case where there is more than one order action type being allowed on the same bar. We are implementing a solution now to the fill timing issue. But even this, does not completely solve the issue. it can still occur due to the chart update interval setting and also when evaluating bars on a close.
