As a rule, you must always clear the trade data out for the symbol and trade account when starting a back test. Otherwise, you are starting off at an uncertain state and can have at least one unmatched fill. And you should be able to reproduce this problem, without even using an automated trading system just simply by submitting the very same orders the automated trading system is doing. If you are able to reproduce this, let us know what you are doing and what existing fills already exist for the symbol and trade account in the Trade Activity Log (make sure you check back years).
