FLATTEN keeps opening new position

For several days every time I Flatten a working order, a new order automatically opens. I'm in Sim mode because I'm worried this issue will persist Live. In effect I always have a position opened, and cannot completely close all positions. If I open my trade orders window or trade positions window, there is nothing there, but each time I open a new position that previously flattened (now opened) position is closed at the same price my new order is opened. I then flatten the new order, which works, but a new position is then opened at the price flattened. Can anyone tell me how to fix this? How to Flatten all positions without automatically opening a new one.

Yes using simulation mode. I don’t want to trade live if there is any chance that this issue will also be present there
