I opened my intraday chart for MSFT and I got the message that the contract description is unknown and would not update. I was able to use this intraday chart in October 2013 and did not have a problem. Now that I opened it again to trade it today, I could not get the intraday chart to update.

You need to use the symbol format documented here which specifies both the exchange and the primary exchange:

I have tried all symbol formats based on your instruction and it just doesn't work. Can you help find out how to get the right symbol format? The weird thing here is I used to be able to receive data just using the symbol MSFT (see attached). Now it just doesn't work. This is the same thing that happens for the stock "FIVE".

CME just got added to the list of symbols that are no longer working that used to work. I used to trade this and now I got the symbol unknown error again. There must be something that causes these symbols not to be recognized. The symbol format instruction does not seem to work for CME, MSFT and FIVE. I noticed these are all Nasdaq stocks. Any idea on how to fix this?

Try this:
MSFT-STK-SMART/NASDAQ

You need to use a similar format for the other symbols you give.

When I try MSFT-STK-SMART/NASDAQ, I get the error file name is not valid. Filenames cannot have the "/" character. I remember I tried this also but got this file name invalid error message.

If you enter the symbol through File >> Find Symbol, the / will be automatically translated for you.

If you use File >> Open Intraday Chart, then use MSFT-STK-SMART[NASDAQ

What am I doing wrong?

What this message from IB is saying is that the NASDAQ part of the symbol is not considered valid.
Invalid destination exchange specified

Please contact IB and ask them what you should set the primary exchange to when requesting market data through TWS. The NASDAQ part of the symbol is considered the primary exchange.

Try ISLAND instead of NASDAQ and ARCA or BATS instead of NYSE.

Adrian, also try updating to the latest version of TWS to rule out TWS as the issue.

My TWS shows the following info:
Build 943.1c Jan 6, 2014.
Java version: 1.7.0_17
OS Windows 8 (amd64, 6.2)
