Shortcut keys malfunction

I have always set my flatten and cancel short cut keys to crtl+alt+q. The last 2 days after my limit was filled and I was in a position I would go to flatten my trade, but the order would stay open and it would cancel my stop loss and limit for target profit. What could be causing this? I checked the short cut keys and everything seems to be fine
