This is usually the result of two things.

1. Not using Good till Canceled for the Attached Orders for the Time in Force:
Attached Orders: Time In Force

2. Or in your case because you are using CQG, if the parent order has not yet filled and Sierra Chart is not running or there is a loss of connectivity, then the child orders will not be sent to the exchange.
