Keep in mind, you can always do what you are asking in code, and just keep track of whether the alert has triggered or not, and then not fire again. But, in terms of setting up an alert for a drawing from a study, you would need to make the drawing a User Drawn drawing, then you can use the standard Drawing Tool Alert options, which have an option for Only Trigger Once.
