stop/flatten keeps opening new position

My orders seem to be backwards. I made a few trades and were fine but now every time a position closes/stops out, a position is opened at that level. I try to flatten but it says no position exists. If you need to get out of a position, check with your broker to ensure that it is closed and that you do not have any open positions. The issue is that you have a missing order fill for some reason. You will need to go through and find the missing fill and then add it manually to close out what is missing.
